What's meant by "first sale" in the following requirement for level up.? "First completed sale/purchase/affiliate sale is at least 30 days old". Does it mean if the requirement is 20 sales, the first sale of 20 orders should be within 30 days?

It's as simple as it says, the first sale has to be30 days old at least. No time frame.
That means If your first sale was 55 days ago and you fulfilled all the other requirements, Then you'll be promoted/level up.
